How to report DSL problem?

-What is the Activation Date:
-How many Jacks:
-Has it worked before:
-Dial tone at Jack:
-Phone plugged into correct jack in wall and on modem:
-Phone cable length:
-Tried other phone cable:
-Tried another phone jack (Regular DSL Only):
-Modem filtered or splittered:
-All phones filtered (Regular DSL Only):
-Unplug all devices except modem (Regular DSL Only):
-Devices near modem causing interference:
-Any changes (electrical storm,CARRIER work in area,renovations):
-Repeat problem (previous ticket opened):
Customer Contact Number:
Customer Availability:(AM/PM/EVE/AD)
